At ProTrainings, we have had a free student video online course and a schools program for over 10 years but we asked our Paramedics, can children learn first aid?

If we consider what the aspects of first aid are, then it’s possible for any age of the child to do something, even if that is alerting the emergency services in the first place. Soon as they recognize that a parent or a sibling or anybody that they are with at the time has become ill, raising the alarm and getting the appropriate level of care to that patient is vitally important. In terms of doing CPR, then clearly there is a physical limitation and we have also got to consider the psychological aspect of a child doing something as aggressive or invasive as CPR. There are other things that children can do that make a big difference and that could be something as straightforward as stopping bleeding, it may be putting somebody into the recovery position to protect the patient’s airway should they vomit and all these measures can make a difference between life and death.

I think if you look at the Denmark model where they have taught kids in junior schools and infant schools right through to university, the survival rate out there now is as high as 25% for cardiac arrests and it’s no accident. I think it is because children pick up things fast, they learn well, they are like sponges, they adapt and by the time they actually get to an age where they can make a serious difference in first aid they are already acclimatized to it. They are not frightened. They are already prepared to do something because they have not had all the myths or the worries that we tend to get in the UK. So I think, yeah, I am a big advocate of teaching school children from a very early age just how to make the triple nine call. Small kids in the UK have already saved lots of lives just by being able to make a triple nine call and explain what they want over the phone. That is the first step, and it builds from there. And I think we are very bad in the UK at encouraging these kids to develop.

At the time of our launch, Sir Bob Russell, Colchester MP and Chairman of the All-Party Parliamentary First Aid Group, said: “I welcome this initiative. Those with knowledge of First Aid play a key role, often in saving lives, in so many ways in treating people who need immediate help because of a fall, cutting themselves, or in other ways injuring or hurting themselves. In Parliament, I continue to campaign for First Aid to be made part of the National Curriculum. In the meantime, I welcome those who are seeking to train young people in life-saving First Aid skills.”

Well, many years on, here we are!

Last week, the Education Secretary (The Rt Hon Damian Hinds MP) underlined the importance of every child having the chance to learn life-saving skills such as CPR, under plans for health education to be taught in every school.

The government’s plan is that all children will be taught basic first aid in schools, under proposals due to be rolled out from 2020.

Education Secretary Damian Hinds said: “Learning the basic skills of first aid and techniques like CPR will give young people the confidence to know that they can step in to help someone else in need and in the most extreme cases – it could potentially save a life.

“That’s why we took the decision to include health education alongside relationship education for primary school children and relationship and sex education for secondary children. These subjects are a crucial part of our work to ensure children learn the wider skills they need to flourish in the modern world.”

Under the proposed new guidance, by the end of secondary school pupils will be taught how to administer CPR, the purpose of defibrillators, and basic treatments for common injuries.

The proposals are part of the Department for Education’s plans to strengthen teaching of health, sex and relationships education – building on free resources already available for schools to teach first aid including those provided by ProTrainings EU.

We’ve always been ahead of the curve when it comes to this topic, as it’s something we care deeply about. ProTrainings has been providing free student first aid to schools using E-Learning with an option of additional practical sessions for years now. GUIDELINES_2015_IMAGEResuscitation Council 2015 recommendations we released last Thursday and we have studied the latest Resuscitation changes and are changing over to the new recommendations on our courses very soon.  The recommendations on CPR change every five years.  On the 15th October 2015, the European Resuscitation Council, ILCOR and UK Resuscitation Council announced the 2015 guidelines.  Our training courses will shortly follow the ERC and UK 2015 recommendations to ensure the latest guidelines are including in our online and classroom courses.

When the rules change, its not that the old rules were wrong, it’s that better advice is now available to improve the survival rates for a patient and improving the understanding of what to do for the rescuer.

There are not as many changes in 2015 as there have been in previous years, but the main changes are as follows.

Guidelines 2015 highlights the critical importance of the interactions between the emergency medical dispatcher, the bystander who provides cardiopulmonary resuscitation (CPR) and the timely deployment of an automated external defibrillator (AED). An effective, co-ordinated community response that draws these elements together is key to improving survival from out-of-hospital cardiac arrest.

The emergency medical dispatcher plays an important role in the early diagnosis of cardiac arrest, the provision of dispatcher-assisted CPR (also known as telephone CPR), and the location and dispatch of an AED. The sooner the emergency services are called, the earlier appropriate treatment can be initiated and supported.

The knowledge, skills and confidence of bystanders will vary according to the circumstances of the arrest, level of training and prior experience. The bystander who is trained and able should assess the collapsed victim rapidly to determine if the victim is unresponsive and not breathing normally and then immediately alert the emergency services. Whenever possible, alert the emergency services without leaving the victim.

The victim who is unresponsive and not breathing normally is in cardiac arrest and requires CPR. Immediately following cardiac arrest blood flow to the brain is reduced to virtually zero, which may cause seizure like episodes that may be confused with epilepsy. Bystanders and emergency medical dispatchers should be suspicious of cardiac arrest in any patient presenting with seizures and carefully assess whether the victim is breathing normally.

With basic life support, the key messages are:

  • Ensure it is safe to approach the victim.
  • Promptly assess the unresponsive victim to determine if they are breathing normally.
  • Be suspicious of cardiac arrest in any patient presenting with seizures and carefully assess whether the victim is breathing normally.

For the victim who is unresponsive and not breathing normally:

  • Dial 999 and ask for an ambulance. If possible stay with the victim and get someone else to make the emergency call. If possible, stay with the victim while calling the ambulance. It has been added that, if the phone has a speaker facility, switch it to speaker mode as this will facilitate continuous dialogue with the dispatcher including (if required) CPR instructions.
  • Start CPR and send for an AED as soon as possible but do not leave the patient to search for an AED. AED units are safe to use even without training so should always be used if available.
  • If trained and able, combine chest compressions and rescue breaths, otherwise provide compression-only CPR. If the compressions are too shallow, it is more harmful than compressions that are too deep, therefore it is re-enforced that compressions should be between 5 to 6 cm in an average adult. CPR is difficult and it remains recommended that if possible change to a second rescuer after two minutes to ensure the quality of compressions.
  • It’s also re-enforced, the importance or recoil. By allowing complete recoil of the chest after each compression results in better venous return to the chest and may improve the effectiveness of CPR. CPR providers should, therefore, take care to avoid leaning forward after each chest compression.
  • It was re-enforced that mouth to nose ventilations are an effective method of delivering breaths if you cannot deliver them via the mouth and it was added that mouth to tracheostomy ventilations we also an effective way of delivering breaths for someone with a tracheostomy tube or tracheal stoma. It was also changed that the maximum interruption in chest compressions to give the two breaths over 1 second and opening the airway each is ten seconds.
  • With children and infants, we say compress the chest by at least one third the depth of the chest but how far is this? Its stated now still as one third the depth of the chest, but clarified as approximately 4 cm for the infant and approximately 5 cm for an older child. We still use two fingers for an infant under 1 year; use one or two hands as needed for a child over 1 year to achieve an adequate depth of compression.
  • With the breaths with children and infants, the duration of delivering a breath is stated as about 1 second, to coincide with adult practice.
  • If an AED arrives, switch it on and follow the instructions.  Minimise interruptions to CPR when attaching the AED pads to the victim. Do not stop CPR unless you are certain the victim has recovered and is breathing normally or a health professional tells you to stop.

Moving on to choking, the rules are the same. Treat the victim who is choking by encouraging them to cough. If the victim deteriorates give up to 5 back slaps followed by up to 5 abdominal thrusts. If the victim becomes unconscious, start CPR.

The 2015 Advanced life support (ALS) guidelines have a change in emphasis aimed at improved care and implementation of these guidelines in order to improve patient outcomes. Increased emphasis on minimally interrupted high quality chest compressions throughout any ALS intervention.  This has also been reflected in the basic life support guidelines.

The rules are reviewed every five years and the last review was in 2010 where the current course material is based.  In 2015 the rules may change but until you know the latest guidelines the old ones are fine. We will put the new videos on the blogs, on company/instructor dashboards and across social media to make sure that all trainers and students understand the changes and what the new guidelines are.

Our USA side have an excellent story on how training pays off in saving a life. As a nurse, Nancy has been trained in CPR many times throughout her career. It was the day after her most recent re-certification that she was called upon to put her skills into use. And it hit close to home.

Just a few weeks ago, Nancy and her family went out for Mother’s Day. During the meal, Nancy noticed her mother acting strangely. It then occurred to her that her mother may be choking. Thinking back to the CPR training she had taken only the day before, Nancy jumped into action and began the Heimlich maneuver and instructed her brother to call 9-1-1. Before he was able to call, Nancy’s efforts were successful and her mother began to breathe again.

Nancy was able to remain calm in what would have been a very difficult situation for most of us. She explained that the actions just came naturally because of the training she had received. Reflecting on the whole experience, and looking back on all of her rescue efforts over her career, this time was different. “When it’s a family member, it kind of puts a new light on it,” she said.

Nancy reiterated to us that she thinks CPR training should be taught more widely, especially in our high schools. “You just never know when you’re going to run into that situation,” she said.  See more stories on Roy On Rescue
